Within the application:
1. Complete all information that applies to the service you are setting up.
2. Select the checkbox next to the type of service that you are setting up.
3. Select the checkbox for e-statements if you wish to receive your bill via email.
4. Print and sign your name and date the application.
***If there is a second member on the application, the secondary SSN and Driver's License fields must be completed. Additionally, both applicants must print and sign their names before the application is submitted.
- Picture of valid Driver's License (for any member listed on the application
- Lease agreement or proof of ownership
NOTE: If you are unable to upload a driver’s license, lease agreement, or proof of ownership, please use your smart phone to take a screen shot and email it to firstname.lastname@example.org.
7. Once you have completed and submitted the application, contact the office at 850-939-2427 to finalize the application.
